Mid-80s Male, Rhinorrhea

This CT case features a mid-80s male presenting with rhinorrhea, and illustrates characteristic findings of Sinonasal Respiratory Epithelial Adenomatoid Hamartoma (REAH) — a benign lesion of the nasal cavity that can mimic more aggressive pathology.

Imaging Highlights

  • Coronal and axial contrast-enhanced CT scans show bilateral hypoattenuating soft tissue centered in the olfactory clefts, with a smooth, polypoid appearance.

  • Sagittal imaging reveals mild bone remodeling and superior nasal cavity expansion.

  • These features are classic for REAH, particularly the widening of the olfactory clefts without cribriform plate erosion.

Diagnostic Insights

REAH is a benign glandular proliferation that typically affects older males, may be unilateral or bilateral, and does not invade critical skull base structures. Recognizing these imaging signs prevents misdiagnosis and unnecessary aggressive treatment.
